South Florida Community Care Network, LLC d/b/a Community Care Plan (“CCP”) is issuing a Request for Information (RFI) for Optometric, Ophthalmological, and Vision Services. A copy of the RFI may be obtained by email to procurements@ccpcares.org or by download at CCP’s NewsRoom at https://ccpcares.org/NewsRoom.
 
Responses to the RFI must be submitted to CCP via email to procurements@ccpcares.org no later than 2:00 p.m., ET, August 26, 2022.
All responses will be analyzed on the basis of the criteria set forth in the Request for Information. CCP reserves the right to accept or reject, in whole or in part and for any reason whatsoever, any and all responses to the Request for Proposal and to waive any formalities in the response process.
